Insurance Coverage and Payment Policies
For clients with Cigna plans, coverage is now available through Wildflower Health.
Sunrise Lactation is subcontracted with The Lactation Network (TLN). TLN provides direct billing services for some BCBS PPO, United, Anthem, Unicare, PNOA and MulitPlan. Patients submit insurance information directly to TLN for verification. Pre-approved visits through TLN are at no cost to you. If your insurance company is not approved by TLN, you are considered a cash-pay patient and will be responsible for paying for fees out of pocket.
Insurance verification must be completed prior to your visit and typically takes about 24 hours. Upon approval you will be required to sign an Assignment of Benefits form with TLN to allow billing of your visit. If you do not complete the insurance verification process and submit the Assignment of Benefits prior to your visit, you will be responsible for paying out of pocket for your visit.
Out of Network Support
Sunrise Lactation is out-of-network for insurance plans outside of The Lactation Network and Wildflower Health. Upon request we will provide you with instructions and a proper medical receipt (super bill) for you to submit to your insurance company for reimbursement. Reimbursement is not guaranteed.
Consultation fees are due at the time of service. Fees are subject to change.
